Forms to be Completed by Managers and/or Parents and kept on record by Team Managers

• Player Medical Form - please have each player fill out a form and keep files on hand for coaches/staff to reference, should the need arise.
• Player's Pledge Form - to be printed and completed by all players and kept on file by manager
• Parent Conduct Form - to be printed and completed by all parents and kept on file by managers

Forms to be completed and submitted to BAHA 

• Parent Social Media Agreement - to be completed and submitted by October 15, 2023
• Player Social Media Agreement- to be completed and submitted by October 15, 2023
• Team Staff - Roster Submission- to be completed and submitted by October 15, 2023
